RYLA Forms

The 2009 District 9810 RYLA will be held from Monday 6 to Saturday 11 July 2009.  The venue will be Lake Dewar Lodge YMCA, 339 Garrards Lane, Myrniong Victoria 3341.

A copy of the 2009 RYLA Application form and associated documentation is available for download via the link further down this page. All relevent sections of this form must be signed by Applicants for this Seminar.

Application Form for RYLA 2009

This form can be completed after downloading and then printed out. The alternative is to print it and complete by hand. Please note that the form consists of 3 sections and requires that both the Applicant and the Nominating club need to supply information.

Section 1 - is to be completed by the Applicant for the RYLA seminar.

Section 2 - is to be completed by the Nominating Rotary Club.

Section 3 - must be retained by the Applicant - this section contains important information about the 2009 RYLA Seminar and should be read carefully.

Sections 1 and 2, and a cheque for $560.00 per applicant should be sent to the RYLA Committee. Address details are in Section 2 of the Application form.
